The goal of this questionnaire is to help us develop goals and objectives, which establish our mid and long-range general strategies. This is an opportunity for our community to express their knowledge and opinions on the services the OBFPD provides and assist in shaping the future of our services.

Please take a few minutes to answer the following questions. With your feedback and assistance, we can get an understanding about customer satisfaction of current services and insight regarding of the public’s willingness to support additional services and if so, does the public support a sales tax to reduce their property tax.

1. Have you or has someone you know, received service during a Fire, Medical Emergency, Vehicle Accident, Boating Accident, or other call for service from the OBFPD?
2. If you have received a service from the OBFPD, how would you rate the delivery of service?
3. If you call 9-1-1 for an emergency (Fire or Medical); from the time of the phone call, how soon would you expect a first unit to arrive on the scene?

How important do you consider the following? 1 is least important 5 is most important.

5. Response time, how quickly the fire truck arrives on scene?
6. Appearance, how the vehicles and the uniforms of the firefighters appear?
7. Safety of the firefighters while doing their jobs?
8. Firefighters that are well trained?
9. The ability to respond to every call for service request (Emergency and Non-Emergencies)?

Please consider the following tax related questions and indicate likelihood to support. 1 Indicates, Definite "No", 5 indicates, Definite "Yes."

The implementation of a sales tax would reduce District-wide property tax obligation by 50% of sales tax revenue received.

10. To maintain current level of service, including replacement of two small brush type fire vehicles, how likely would you be to support a ¼ cent sales tax, with estimated revenue of $1.5M, reducing property tax obligation by $750,000?
11. To increase services to implement two staffed EMS quick response vehicles, how likely would you be to support a ½ cent sales tax with estimated revenue of $3M reducing property tax obligation by $1.5M?
12. To maintain the current level of service, how likely would you be to support a 15 cent per 100 dollars of assessed valuation levy increase on property?
13. To increase services to implement two staffed EMS quick response vehicles, how likely would you be to support a 30 cent per 100 dollars of assessed valuation levy increase on property?
